We have been selected as a PHASE 1 winner of the HRSA Building Bridges to Better Health: A Primary Health Care Challenge - to improve health outcomes for health center patients and improve care coordination between local providers and social service organizations.

Fueled by a recently awarded $1 million grant from the U.S. Centers for Disease Control and Prevention, Iowa State University researchers, the Iowa Community HUB and community partners will study new strategies to help reduce risks of falls in older adults.
