Iowa Falls Prevention Coalition

The mission of the Iowa Falls Prevention Coalition is to facilitate collaboration between state, community and health care system partners to reduce falls by building awareness, providing education, and supporting implementation of fall prevention efforts.

Coalition Membership

The Iowa Falls Prevention Coalition is made up of 70+ Iowa agencies and community-based organizations working together to support the expansion and sustainability of falls prevention programs and services across the state that educate Iowans about the risk of falls, address their fear of falling, and teach exercises to improve their balance.

Iowa Falls Prevention Coalition Charter

The Coalition supports maintaining and expanding fall prevention programs and services in the state of Iowa that focus on communities, hospitals, clinics, and other outpatient settings.

Coalition Partner Commitment Form

Coalitions can bring together organizations and providers to collaborate to reduce falls, identify state or community needs, recommend policy changes, and build capacity.

The Iowa Falls Prevention Coalition's 3-Year Action Plan outlines key goal areas and objectives, which are being advanced through cross-sector collaboration with partners from across the state.

Falls Prevention Data Brief

The Iowa Department of Health and Human Services releases an annual Iowa Falls Prevention Data Brief. The data brief highlights the impact of falls in Iowa and strategies to reduce fall risk in our state.

The 2023 Iowa Trauma Registry Report

The Trauma Registry is the primary vehicle for data collection and management related to patients that presented to a hospital for the care of one or more injuries in any given year in Iowa. Overall, falls do seem to be driving the rate of injury resulting in ED/hospital cases in Iowa.

2024 HUB Landscape Assessment Report for Falls

This brief report is designed to provide critical information regarding the scope and reach of AAEBIs and other falls prevention activity around the state.

Iowa Falls Screening Program

Increase the number of clinical providers and community partners conducting falls screening across Iowa using the CDC's Stopping Elderly Accidents, Deaths & Injuries (STEADI) Initiative and referring to the HUB for falls prevention interventions.

STEADI Initiative

CDC's Injury Center created this initiative for healthcare providers and community partners who work with older adults and individuals with disabilities who are at risk of falling, or who may have fallen in the past.

STEADI Fall Screening Course

Learn how to conduct fall risk screening to help address falls in your clinic or community. The FREE online training course takes about 2-3 hours to complete and covers the key components of the CDC STEADI fall risk screening protocol.

STEADI Brochure - Falls Questionnaire

Have individuals complete the questionnaire to find out their fall risk. If they have a fall risk, encourage them to contact their health care provider and the HUB to be connected to a falls prevention program.

ASTHO Project

ASTHO (Association of State and Territorial Health Officials) plays a key role in falls prevention by developing and disseminating resources, best practices, and guidance to state and territorial health agencies.

ASTHO is focused on expanding fall prevention strategies across the country, particularly focusing on community-clinical linkages and surveillance to identify high-risk populations.

This guide is intended to be a comprehensive resource for state health departments in understanding how to align current falls prevention efforts to support community-clinical linkages and offer strategies to assist in implementing new efforts to support these linkages

Local Falls Prevention Coalition Efforts

The State Coalition encourages and supports the development of local falls prevention coalitions and engages them in partnership opportunities.

2025 National Falls Prevention Awareness Week

September 22-28, 2025 is Falls Prevention Awareness Week, a nationwide observance to raise awareness on preventing falls, reducing the risk of falls, and helping individuals live without fear of falling.
