Iowa Community Corps Member Application

The Iowa Community HUB and Grace Fitness are working together through the SNAP-Ed HEAL grant to improve community health by expanding access to nutrition, physical activity, and support programs, with a focus on sustainable, community-driven change.

The Iowa Community HUB has been selected by the CDC for an evaluability assessment to improve its lifestyle change interventions, focusing on increasing physical activity and nutrition in underserved communities.

Iowa Public Health Association recognizes the Iowa Community HUB with the 2024 Iowa Public Health Association Partner of the Year Award. 

The Iowa Community HUB was selected as one of 20 grantees nationally to be awarded funding through USAging's Center of Excellence to Align Health and Social Care (COE).
