Mapping the Patient Journey: Building More Connected, Equitable Care

Mapping the Patient Journey shares how Primary Health Care and the Iowa Community HUB worked together to better understand and improve patient engagement across the full continuum of care. Developed through the IHI/Pfizer Collective Action Collaborative, the journey map visualizes patient emotions, barriers, and decision‑making from wellness screenings through follow‑up, with a focus on chronic disease prevention, health equity, and the impact of social determinants of health.

The project aimed to identify key touchpoints where nurse care managers, community health workers, and navigators can reduce barriers, improve communication, and strengthen coordination between clinic and community systems. By applying these insights, the journey map supports more engaging, trusted, and equitable care workflows that help patients stay connected, informed, and supported throughout their care experience.
