Our Purpose:

The purpose of the Iowa Community HUB is to prevent and manage chronic disease across Iowa by connecting individuals of all ages, abilities, incomes and lived experiences, to community health programs and support, ensuring everyone has access to resources and opportunities to lead healthy lives. 

We accomplish our mission by supporting community-based organizations like you, in your implementation, expansion, and sustainability of quality programs and services.

Who is a HUB Member?

Community-based organizations that deliver chronic disease prevention and management programs. We are especially interested in organizations offering evidence-based programs.

What are Evidence-based Programs?

Evidence-based programs are structured, proven interventions that have been tested and shown to improve health outcomes, such as the Diabetes Prevention Program, Tai Chi for Arthritis and Falls Prevention, Better Choices, Better Health, and Walk with Ease.

We also Support Organizations Delivering Evidence-Informed Programs

These programs are grounded in research showing promise for improving health outcomes, but have not necessarily undergone the same level of rigorous testing and standardization as formal evidence-based programs.

Billing and Claims Reimbursement

Get connected to a healthcare payment system, with support for data reporting, billing and claims reimbursement, and contracting opportunities with healthcare payers.

Compliance Support

Receive support with data integrity, HIPAA training and security, meeting contractual obligations, and fostering a culture of compliance.

Referral Network

Get assistance in developing referral pathways into your programs through the HUB’s clinic-community connections.

Funding Opportunities

From collaborative grant opportunities to healthcare payment contracts and innovative braided funding sources, we identify and direct funding to community-based organizations (CBOs).

Program Promotion

The HUB works with connector partners across Iowa to promote community partners' programs statewide through established communication systems.

Training and Resources

Access free and discounted training and professional development opportunities, along with state and national resources to enhance program outcomes.

Aggregate Data

Access aggregated data from statewide program outcomes to guide your program management and decision-making.
