Our HUB Navigators are here to help people find their way to the support they need. They listen, guide, and connect individuals to programs and resources that can make everyday life healthier and easier.

What is a HUB Navigator?

Our HUB Navigation services connect individuals to programs and resources that best meet their needs. Referrals are received by HUB Navigators, who are Community Health Workers trained to provide personalized support. Navigators work one-on-one with each person to identify the program that is the best fit and screen for both health-related and everyday needs.
Since 2021, HUB Navigators have helped enroll more than 4,500 individuals in virtual and in-person programs, improving access to care and community-based supports.

Essential Functions of a
HUB Navigator

HUB Navigators encourage and support people in making healthy choices and connect them to evidence-based programs and community resources. They assist with program registration and paperwork, and make referrals to social care services when additional support is needed.
Navigators follow up to make sure individuals stay enrolled and engaged in programs. They also collect survey information to better understand health needs and outcomes, and work with partner organizations to coordinate care.

HUB Fresh Produce Box Program - Food + Health, Delivered


If an individual needs access to fresh produce, HUB Navigators can enroll them in the HUB Fresh Produce Box Program for monthly home delivery of mixed fresh produce while they participate in a qualifying health program. Check out the flyer for more details.
