What is the Iowa Community HUB?

The Iowa Community HUB is a statewide community care hub (CCH) dedicated to connecting communities with health promotion programs and addressing health-related social needs through a network of collaborative partners.

Connecting Healthcare and Community

As a Community Care Hub, the Iowa Community HUB connects healthcare and community partners to make it easier for Iowans to access programs that help prevent and manage chronic conditions, address health-related needs, and connect to resources and support that promote better health and independence.

Community Care Hubs are growing nationwide as a way to strengthen connections between healthcare and community-based services. Watch this short video to learn how Community Care Hubs work and why they matter.
