Iowa Community HUB Earns Age Friendly Recognition
The Iowa Community HUB is proud to announce that it has received Age Friendly Recognition through the Age Friendly Public Health Systems (AFPHS) initiative. This recognition highlights our commitment to making healthy aging a core priority across our work and strengthening the systems that support older adults in our communities.
Age Friendly Recognition in public health is awarded to organizations that integrate older adult health into their foundational services. It reflects a systemwide commitment to advancing healthy aging through collaboration, policy development, prevention, and coordinated community-based care.
The Age Friendly Public Health Systems initiative, led by Trust for America’s Health, recognizes health departments and partner organizations that implement the six Cs framework. These core actions include creating policy and environmental changes that support healthy aging, connecting partners across sectors to address social determinants of health, coordinating existing supports and services, collecting and using data to understand older adult needs, communicating clearly to promote health and well-being, and complementing existing programs that support healthy aging.
Together, these strategies help ensure that older adult health is treated as a priority across public health systems and community partnerships. By strengthening coordination and improving how services are delivered, AFPHS recognition supports better outcomes and a more connected system of care.
As the population of older adults continues to grow, this recognition reflects an important step forward in reducing fragmentation, improving prevention, and ensuring that older adults have access to the resources and support they need to remain healthy, independent, and engaged in their communities.
