Iowa Community HUB Selected for CDC Evaluability Assessment on Lifestyle Change Interventions
The Iowa Community HUB (HUB) has been selected by the Centers for Disease Control and Prevention (CDC) for an evaluability assessment (EA) as part of their initiative, Identifying and Evaluating Strategies to Improve Implementation Outcomes of Lifestyle Change Interventions (LCIs). Out of over 100 nominations, the HUB stood out for its commitment to improving physical activity and nutrition within the communities we serve.
This opportunity will allow the HUB to collaborate with CDC and its contracted partner, ICF, in a thorough assessment of the HUB’s LCI improvement strategies, including:
- Connecting individuals and families to a menu of evidence-based LCI options based on their needs, preferences, life circumstances, and readiness levels.
- Implementing LCIs that have flexible enrollment programs (e.g., rolling enrollment, flexible eligibility, flexibility to join classes at different times, flexible delivery modalities).
- Addressing social determinants of health and health-related social needs of populations of focus through screening, referral, and/or delivery of services within LCIs.
- Employing innovative social media marketing strategies to increase awareness of LCIs among populations of focus.
- Engaging in community-level marketing of LCIs through community-based organizations, churches, senior centers, community health workers, care coordinators, and healthcare providers.
- Enhancing financial sustainability for LCIs dedicated to underserved communities through strategic alliances and improved resource coordination.
- Working with payers to establish reimbursement rates and reimbursement models that make LCI delivery sustainable for implementing organizations.
Through an in-person site visit in the Spring of 2025, evaluators will engage with HUB leadership, staff, and stakeholders to gain insights into our implementation strategies, capacity, and data collection efforts. The process will culminate in a debrief session where we will receive valuable feedback, key takeaways, and a tailored theory of change model to showcase and enhance our impact.
Beyond the immediate benefits of the EA, the HUB may also be considered for a subsequent case study or rigorous evaluation in 2026-2027, offering even greater opportunities for growth and recognition.
