Trina Radske-Suchan is the President and CEO of the Iowa Community HUB, a nonprofit organization that serves as a community care hub (CCH) to support partners across Iowa. With more than 20 years of experience as a physical therapist in central Iowa, Trina has played a pivotal role in leading statewide health initiatives that address chronic disease, reduce health disparities, and promote falls prevention for aging populations. As Co-Chair of the Iowa Falls Prevention Coalition, she has been instrumental in advancing falls prevention efforts across Iowa.
Since the launch of the Iowa Community HUB in 2020, Trina has led the organization in its mission to initiate, expand, and sustain health and wellness initiatives that address essential health needs and create lasting community impact. Under her leadership, the HUB has gained national recognition as a standout CCH. In 2024, the organization was awarded funding from USAging’s Center of Excellence to expand its work in coordinating access to social care programs, specifically addressing health-related social needs.
The HUB’s efforts have been recognized with several honors, including the Iowa Medical Society’s 2023 Washington Freeman Peck Award for care coordination, quality, and patient outcomes. In 2024, the HUB also received the Partner of the Year Award from the Iowa Public Health Association, highlighting its ongoing contributions to the community.

As Director of Operations at the Iowa Community Hub, Rachelle brings over 20 years of proven leadership and management experience, including working with historically underrepresented groups, refugees, and immigrants. Backed by a Bachelor of Science in Marketing and a Master of Business Administration (MBA), Rachelle leverages expertise in strategic planning, operational efficiency, and program development to advance the Hub’s mission: fostering equity and creating sustainable solutions that improve the health and well-being of Iowans by addressing the social determinants of health.
In previous roles, Rachelle has successfully led initiatives that prioritized inclusivity, capacity building, and access to critical resources. At Iowa Workforce, she oversaw multiple programs such as the Disabled Veterans Opportunity Program (DVOP), the Work Opportunity Tax Credit Program (WOTC), and Migrate and Seasonal Farm Workers (MSFW). This work included providing critical support, resources, and training to empower diverse populations. A noteworthy achievement includes founding the Financial Empowerment Forum for Women, which empowered individuals to achieve economic stability. Certified as a Master Practitioner in Financial Capabilities, Rachelle brings unparalleled expertise in financial education and empowerment to underserved populations. Additionally, certified in advanced leadership, financial counseling, diversity, and project management, Rachelle is skilled in managing multifaceted initiatives, ensuring high-quality outcomes and measurable impact.
Guided by the vision of the Iowa Community Hub, Rachelle partners with community organizations supporting evidence-based programs addressing chronic disease, health equity, and wellness. By linking individuals with essential services, Rachelle contributes to creating a healthier, more inclusive Iowa where all residents can thrive.

Jocy K is a graduate of ISU with a Bachelors of Science in Psychology and a minor in Women’s Studies. Currently, she is wrapping up her Master’s in Clinical Counseling. Fluent in English and Spanish, with a working knowledge of Portuguese, Jocelyn’s linguistic versatility enhances her ability to connect with diverse communities. Her professional journey connects her passion for mental health with holistic wellness. Alongside her academic pursuits, Jocelyn is on the verge of completing her Integrative Nutrition Coaching certificate, expanding her expertise in fostering health and wellbeing.
In her role as a HUB Navigator, Jocelyn focuses on collaborating with care teams, engaging with the community, and connecting individuals to community programs and resources.

Rita graduated from Grandview University in the spring of 2020 with a BA dual-degree in Human Services and Psychology and a certificate in Industrial Organizational Psychology. Rita has received her Community Health Worker certification through Iowa Chronic Care Consortium. With her experience in the Human Services & Psychology fields and current position within the HUB, Rita aspires to earn a Masters degree in the future. As a HUB Navigator, Rita will work with referred individuals and our program partners to support and help deliver excellent health and wellness resource connections statewide.

Adria Huseth, RD, LD, CPT

Nutrition and Wellness Outreach Coordinator
Adria Huseth is a registered, licensed Dietitian and Personal Trainer. She was a former manager of Nutrition Communications and Research for the National Pork Board where she managed human nutrition research & communications and education outreach for ten years. Before joining the National Board in 2010, Huseth worked for the government nutrition education program WIC (Women, Infants and Children) in Central Iowa. Adria graduated from Iowa State University in 2008 after she completed the Didactic Program in Dietetics and joined Florida Hospital for her 10-month CADE dietetic internship. She also received a Bachelor of Arts degree in Marketing from the University of Iowa in 2004 and her personal training certificate from National Academy of Sports Medicine (NASM) in May of 2016. Adria has her Certificate of Training in Adult Weight Management and has completed the Integrative and Functional Nutrition Program.
Adria is a member of the Academy of Nutrition and Dietetics including the Food and Culinary Professionals, Public Health and Community Nutrition and Dietitians in Business and Communications DPG’s. She is also a member of the Iowa Dietetic Association and Integrative and Functional Nutrition Academy.
Adria is passionate about helping others live a healthful lifestyle from the inside out while staying abreast of current human nutrition research. She resides in Urbandale, Iowa where you can find her encouraging her family to try new healthy dishes she is whipping up in her kitchen!
